How Much Notice is Necessary: 6-8 weeks notice for new clients, otherwise 3 weeks notice is needed
Eligibility: Clients must have active Medicaid and the DHHS-5330 Medical Verification for Transportation Form on file. a) If client has active Medicaid but does not have an HMO we can assist with transportation to all non-emergency medical appointments b) If client has active Medicaid and an HMO we can only assist with transportation to mental and dental health appointments c) If client has active Medicaid/HMP and also has an HMO we can only assist with transportation to mental health appointments.
