MDHHS does not provide transportation, but does provide reimbursement to providers who transport eligible MA clients. Reimbursement is provided at the least expensive cost/means.

Eligibility: Clients must have active Medicaid and the DHHS-5330 Medical Verification for Transportation Form on file.
- If a client has straight Medicaid, DHHS can assist with transportation to non-emergency medical appts.
- If a client has a provider for the Medicaid, they will need to contact the provider for all non-emergency medical transportation.
