MDHHS does not provide transportation, but does provide reimbursement to providers who transport eligible MA clients. Reimbursement is provided at the least expensive cost/means.

How Much Notice is Necessary: 6-8 weeks notice for new clients, otherwise 3 weeks notice is needed
Eligibility: Clients must have active Medicaid and the DHHS-5330 Medical Verification for Transportation Form on file.
- a) If client has active Medicaid but does not have an HMO we can assist with transportation to all non-emergency medical appts.
- b) If client has active Medicaid and an HMO we can only assist with transportation to mental health or substance abuse appts.
- c) If client has active Medicaid/HMP and also has an HMO we can only assist with transportation to mental health appts.
- d) Healthy Kids Dental – regardless if the client has an HMO or not.
